About the team we are recruiting to

Coventry and Warwickshire Clinical Integrated Care Board were awarded Vanguard status for the West Midlands via the NHS England Framework for Integrated Care. The Framework developed, named by the children and young people (CYP) as Positive Pathways, has been developed in collaboration with young people from across Coventry and Warwickshire (C&W) as a response to the NHS England & NHS Improvement Long Term Plan (LTP) commitment to provide additional support for the most vulnerable CYP. There are four cohorts of young people that will benefit in Coventry: CYP open to the Youth Justice Team, CYP open to Edge of Care, CYP returning to the community from the Secure estate and CYP who have a school attendance of less than 50%. This funding enables the development of innovative working practices and collaborations that stretch across traditional agency boundaries, promotes genuine integration and co-production. A system trauma mobilisation plan has been developed underpinned by a Trauma Needs Analysis and is moving at pace to co-produce services from a range of statutory and voluntary agencies, developing and curating a universal and targeted Children and Young People’s Social Prescribing offer called Positive Pathway that the Positive Directions Practitioners will use as a tool to promote health and well-being seeking behaviours.

What is the job role?

We are recruiting for 5 posts currently available in our team. The first is an education support worker, alongside 4 x support workers, all on a full-time fixed term contract basis for 18 Months.

Edge of Care is a service that supports young people at risk of entering care or custody to stay with their families. We use evidence-based, models of practice and assist families to make significant, positive changes.

You will provide intensive support to young people aged 8 – 18 and their families, usually over a period of 6 – 12 weeks. The work builds on a family’s strengths and introduces coping strategies. It mixes practical support with strong challenge, to address complex and enduring needs.

Team members engage young people and their families to defuse crisis within the home. They assess problems, develop measurable goals and help the whole family acquire the skills necessary to achieve them.

Cases will span a wide range of issues including family conflict, absence from home, sexual exploitation, educational issues, anti-social behaviour, criminal exploitation, and substance abuse. Working closely with other agencies and local services, you will be on the front line of an approach that improves behaviours, relationships, confidence, aspiration, employability, living conditions, mental and physical health.

As a Trauma Informed Practitioner on the Positive Directions Programme, you will take on a portfolio of cases, working directly with young people and families in their homes, schools, police custody etc.

Providing a flexible crisis response service with safeguarding at its heart, you’ll complete an initial assessment and develop strong skills in delivering intensive interventions.

This post is exempted under the Rehabilitation of Offenders Act 1974 and as such appointment to this post will be conditional upon the receipt of a satisfactory response to a check of police records via Disclosure and Barring Service (DBS).

Who are we looking for?

We are looking for practitioners to rise to the challenge of supporting children and young people to safely remain at home.

In this role you will be working with Children and young people open to children’s social care services.

It is essential that you bring experience of working with “Children & Families” and it would be desirable for you to hold or work towards a Level 3 NVQ in Children & Young people’s work.

EOC keep Children and Young People at the Heart of all our work, we listen, respond and value our families and communities.

This is an exciting opportunity to have a role in combining direct practice and contribute to the Positive Direction’s Programme and enhancing professional development.

Are you innovative, positive, dedicated and not afraid of change or challenge? Then help us to transform the lives and futures of children & young people on the Positive Directions Programme.

You will need to be passionate about working with Children, Young people and their families and have a reassuring presence, be calm in a crisis and able to work with families in their homes, helping them make the necessary changes to stay together.

You are a team player with a flexible approach, willing to work on a shift basis that may include evenings and weekends.
